Rotary Club Dimapur under \u2018Gift of life\u2019 project would be sponsoring children suffering from congenital heart disease and in need of cardiac surgery.
\nThe sponsorship would depend on the age of the child below 12 years of age, the gravity of the disease and the economic status of the child\u2019s family.
\nAny child suffering from heart disease have been informed to submit application for sponsorship to the president, Rotary Club of Dimapur, Rotary Centre, Midland, Dimapur and to contact 9436061260 (president or 9435701216 (co-ordinator) for more details.
\nDocuments to be submitted include: name of the child, age and gender, father name and his residential\/ permanent address, mother name.
\nPermanent Address with supporting document duly signed by village\/tribal council\/GB, annual income certificate (Affidavit) duly attested by 1st Class Magistrate in `20\/- judicial stamp paper, no objection certificate from parents in form of affidavit to the effect that they have no objection to under take heart surgery and treatment of their child at their own risk and responsibility.
\nThis certificate is to be made in `20\/- judicial paper and duly attested by First Class Magistrate.
\nFour copies of recent passport size photograph, echo- cardio report in original, Contact person and their telephone numbers and name of attendants who would accompany the child to New Delhi with their address and contact information. <\/p>\n
